What is Depression?

Depression, also referred to as Major Depressive Disorder (MDD) or clinical depression, is a common and serious mood disorder characterized by persistent feelings of sadness, hopelessness, and a loss of interest or pleasure in most activities. It affects how a person thinks, feels, and functions in daily life.

Depression is more than occasional sadness. According to the DSM-5 (APA, 2013), a diagnosis of Major Depressive Disorder requires the presence of at least five symptoms for two weeks or longer, one of which must be either depressed mood or loss of interest/pleasure.

These symptoms must represent a change from prior functioning and cause clinically significant distress or impairment in social, occupational, or other important areas of life.

Core Symptoms of Depression

Symptoms may include:

  • Persistent sad, empty, or anxious mood
  • Loss of interest or pleasure in hobbies and activities
  • Feelings of hopelessness or pessimism
  • Irritability, frustration, or restlessness
  • Fatigue or decreased energy
  • Difficulty concentrating, remembering, or making decisions
  • Sleep disturbances (insomnia or hypersomnia)
  • Appetite or weight changes
  • Feelings of worthlessness, guilt, or helplessness
  • Physical symptoms without a clear cause (e.g., headaches, digestive issues)
  • Thoughts of death or suicide, or suicide attempts

Not everyone with depression experiences all symptoms. Some may have somatic complaints, while others may feel emotionally numb or disconnected.

Types of Depressive Disorders

Major Depressive Disorder (MDD)
This is the most widely recognized form of depression and involves episodes of severe symptoms that interfere with daily functioning.

Persistent Depressive Disorder (PDD, formerly dysthymia)
PDD is characterized by a depressed mood lasting at least two years (or one year for children and adolescents). Symptoms are less severe but more chronic and may include low self-esteem, fatigue, and irritability.

Postpartum Depression
A form of depression that affects some women after childbirth, involving intense sadness, anxiety, and exhaustion.

Seasonal Affective Disorder (SAD)
SAD is a subtype of depression triggered by seasonal changes, most commonly emerging during the fall and winter months.

Premenstrual Dysphoric Disorder (PMDD)
A severe form of premenstrual syndrome that includes mood swings, irritability, and depression during the luteal phase of the menstrual cycle.

Depression In different Populations

In Adults
Adults may experience depression as a loss of motivation, chronic fatigue, irritability, and withdrawal from relationships. It often co-occurs with anxiety, substance use, and chronic pain conditions.

In Adolescents
Teen depression may manifest as irritability, academic decline, social withdrawal, or somatic complaints. It’s sometimes mistaken for “typical teenage behavior,” delaying diagnosis.

In Older Adults
Symptoms in older adults may be misattributed to aging or physical illness. Watch for unexplained aches, memory problems, or withdrawal from social engagement.
